All team members are required to fill out a mission sign up form 2 months prior to trip departure. Medical and Volunteer releases must also be filled out by each team member and are due one month before departure.
The insurance form is only required for international teams, and only one form (or however many necessary for your group’s size) is needed per team. The only part of the form that needs to be filled out (and must be filled out completely) is the chart. Please do NOT put fill out the billing section with your personal or church’s information. Must be turned into H&F office at least 2 weeks prior to trip or no coverage will be offered.
The affidavit for minors is required only on international trips where a minor is traveling without his/her legal guardian(s).
